Vision of Faith By Laws 1. Applications are taken in the order we receive them. After reading each document THOROUGHLY please print out the application and rules, and after both spouses have signed each document, send them both in SIGNED AND COMPLETED so that your application can be processed quickly and avoid any delays. Vision of Faith Reversal Ministry is here to help you fund your reversal, and because others will be helping you to finance it as well, we need to ensure that the rules of the group are upheld by all members and we know that we are helping people that are honest. You will be notified of your position on the waiting list once your application has been received and accepted. 2. All applications are subject to review by our Ministry Board. The President and Officers will make the final decision on all applications. While we prefer to fund MARRIED COUPLES, an exception may be made for ENGAGED COUPLES and SINGLE WOMEN. (This will be subject to approval by the President - please contact Tia Daugharty.  All applications will be reviewed. 3.ONCE YOUR APPLICATION HAS BEEN APPROVED, AND YOU HAVE BEEN ADDED TO THE WAITING LIST, YOU WILL BE REQUIRED TO POST TO THE MESSAGE BOARD A MINIMUM OF 3 TIMES PER WEEK, INCLUDING THE MONDAY ROLL CALL, THANKFUL WEDNESDAYS AND FRIDAYS – WEEKEND PLANS.  THIS IS NOT AN OPTION. While we realize that you are busy with your personal life, we truly desire and strive to share in your journey - as well as offer encouragement and support along the way. We want the members of the group to know you so that we can all offer genuine support as your journey continues.  This also creates a connection between the members on the waiting list as well as those who are not.  If the connection is there, others will want to help the fund grow, and therefore, helping each member on the waiting list to get the funds faster! You will also need to participate in all Fundraisers submitted to board as well as come up with ideas we can use to help raise funds. We feel if we all work together it will help funding to go faster. NOTE: IF YOUR SCHEDULE PROHIBTS YOU FROM POSTING UNDER THESE GUIDELINES, CONTACT AN OFFICER FOR ARRANGEMENTS. 4. ALL applicants are expected to send an initial $25.00 at the time the application is submitted. Thereafter, a MINIMUM monthly donation of $25.00 is required which will be applied to the reversal accounts. The monthly fee will be due on the 15th of EACH month. If at any time you desire to send more than the minimum amount, please feel free to do so, as this will help the list to move along quicker and funds to be completed faster. These payments are non-refundable and are made with the understanding that the ONLY way this group can continue to operate is if we keep the funds moving and get each fund completed. Vision of Faith Reversal Ministry will grant a MAXIMUM of $3,000 per reversal surgery. IF your reversal physician charges more than the $3,000 LIMIT, YOU will be responsible for the additional amount. Furthermore, any additional fees over the group limit which you are responsible for must be acquired by you BEFORE the fund reaches the maximum of $3,000. Vision of Faith will only fund a maximum of 3000.00.   NOTE: ANY MEMBER WHO IS MORE THAN 10 DAYS LATE (NO LATER THAN THE 25TH OF EACH MONTH) WITH PAYMENT WILL BE REMOVED WITHOUT NOTICE, UNLESS THEY CONTACT AN OFFICER TO EXPLAIN THEIR SITUATION AND MAKE ARRANGEMENTS.  NO EXCEPTIONS! PLEASE NOTE BECAUSE A DEPOSIT IS OFTEN REQUIRED SEVERAL MONTHS IN ADVANCE OF SURGERY (WHEN THE SURGICAL DATE IS SET), IT IS THE MEMBERS FULL RESPONSIBILITY TO PAY THE DEPOSIT. VISION OF FAITH WILL NOT BE RESPONSIBLE FOR ANY DEPOSIT OR FEES ASSOCIATED WITH BOOKING YOUR SURGERY. BECAUSE THESE DEPOSITS ARE GENERALLY NON-REFUNDABLE, WE ASK THAT EACH MEMBER CAREFULLY CONSIDER THE BEST TIME TO BOOK YOUR PROCEDURE. 5. Please understand that the mission of Vision of Faith is to assist in funding, and that is why we exist. We completely understand that life is filled with unexpected events and emergencies will arise. In the event that something unexpected happens, and you are unable to meet your obligation of the required EXPECTED MINIMUM monthly donation, Please E-mail Tia, Heather or Shannon and they can work with you.  We are here to help, not judge! In the event a member is removed from the list for nonpayment the member CAN reapply for funding.  They will need to submit a new application and be placed on the bottom of the waiting list.  In other words, they will be starting over.  6. It is REQUIRED that ALL applicants who receive funding by the group REMAIN members and CONTINUE to contribute until an amount of 1/2 of the money funded to them has been repaid. This repayment of funding monies will be contributed back into the funding program to help fund those individuals who contributed to your reversal surgery. Just as others have extended to help you, please return the kindness. We will list your name on our website to show the couples we have funded. 7. Please be aware that all donations are NON-REFUNDABLE! If you are removed from the list for any reason all funds you have donated will be applied to the funding of the next person on the list and will NOT be returned to you. 8. Regular updates will be posted to keep everyone informed of the progress made on the funding program. In addition, a waiting list will be available so that each member seeking funding can see their exact placement on this list. Furthermore, we will routinely keep the group aware of the current amount needed to complete each fund. Please let us know if there is a change in your status (such as coming up with your own funding), or a desire to be removed from the list, or the doctor you choose on your application has been changed,etc. We strive to have good communication with all of you. Vision of Faith is not just a funding ministry - we are also here to provide emotional support and encouragement. If for some reason you change anything on your application it will be your responsibility to make us aware of it to ensure when it’s your time for surgery things run smoothly. 9. For your convenience, minimum monthly donations, as well as any other donations, can be sent in 2 ways: Pay Pal and money orders. To contribute via Pay Pal, simply click on the Pay Pal button on the homepage. Money orders should be mailed and made out to: Heather Walter Contact her by e-mail to obtain her home address you are mailing in your INITIAL donation, you MUST include your SIGNED and COMPLETED application and SIGNED copy of the RULES along with your first initial donation. Please remember the faster the application is received, the higher position you will have on a funding list. 10. Please be aware that Vision of Faith Reversal Ministry is not a group who has a Non-Profit status. As of now we are NOT a 501(c) 3 tax exempt Organization; THEREFORE, donations are NOT tax deductible. We plan to do so in the future. 11. Be advised that as Vision of Faith grows and evolves, it may/will become necessary to update/change the rules, and as a member, you AGREE to be bound by the latest rules as posted to the group's website www.vision-of-faith.org and to the e-mail loop as well. You will also be responsible for checking the rules on the message board as they will always be updated first. 12. While we do not mind if you are in another TR/VR support group, we WILL NOT approve any member for funding if they are currently on another funding list. It is our belief that if you have to send in funds for more than one group it could cause you financial hardship at some point. 13.  Vision of Faith funds both Vasectomy Reversals and Tubal Ligation Reversals. In order to remain fair to all members, ALL DONATIONS and FUNDRAISERS will be equally distributed to both funds. In the event that one fund has no members waiting to be funded, ALL monies collected will be distributed into the fund with waiting members. We have read the three pages of rules consisting of 13 items and agree to be Bound by the terms listed therein. We are aware that no guarantees for funding are being made. We understand and acknowledge that Vision of Faith Reversal Ministry is not a loan company and will in no way be responsible for anything that happens in the office of the doctor, nor any complications arising during or after surgery. In short, Vision of Faith Reversal Ministry will not be held liable for any type of medical issue, nor further financial obligation exceeding the funding provided to members at the time of surgery.
